Write a paper (1,000-1,500 words) in which you apply the concepts of epidemiology and nursing to research a communicable disease. Refer to "Communicable Disease Chain," "Chain of Infection," and the CDC websites, located in the topic Resources, for assistance when completing this assignment.

Describe the chosen communicable disease, including causes, symptoms, mode of transmission, complications, treatment, and the demographic of interest (mortality, morbidity, incidence, and prevalence).

Explain why this is a reportable disease. Provide details about reporting criteria. 

Describe the social determinants of health and how those factors contribute to the development of the chosen communicable disease. 

Discuss the epidemiologic triangle as it relates to the communicable disease you have selected. Include the host factors, agent factors (presence or absence), and environmental factors.

Identify any special considerations or notifications for the epidemiologic triangle (community, schools, or general population).

Explain the importance of demographic data to community health.

Describe the role of the community health nurse in primary, secondary, and tertiary prevention through tasks such as case finding, reporting, data collection, analysis, and follow-up. Provide an example of how Christian worldview can be integrated by a community health nurse when working with a population impacted by the communicable disease selected.

Identify at least one national agency or organization that addresses the communicable disease chosen and describe how the organizations contribute to resolving or reducing the impact of disease.

Discuss a global implication of the disease. How is this addressed in other countries or cultures? Is this disease endemic to a particular area? Provide an example.

A minimum of three peer-reviewed or professional references (i.e., professional health organizations like CDC, WHO, OSHA, DHS) is required.

The purpose of this assignment is to plan and develop an outline for your community teaching plan and gain feedback from a community health representative in your local community. The teaching plan developed in this assignment will be used to develop your Community Teaching Presentation due in Topic 5. Complete the following:

Identify the demographic that you would like to give your presentation to and then select a community site where that demographic can be found.

Complete the "Community Teaching Acknowledgement Form," found in your topic Resources. This form requires a handwritten signature from the appropriate representative selected, allowing permission for you to teach your selected topic at that site.

Complete the "Community Teaching" template resource, found in your topic Resources. This form will help you organize your teaching plan and create an outline for the written assignment due in Topic 5.

Review the plan with the representative that signed the "Community Teaching Acknowledgment Form" and request feedback (strengths and opportunities for improvement). The feedback received will be used to refine the teaching plan in Topic 5.

The purpose of this assignment is to conduct a community assessment in a form of an interview and physical appraisal of the community.

Part I: Assessment

Based on the demographic and community selected in the Topic 3 assignment, perform a physical assessment of the community site using the "Functional Health Patterns Community Assessment Guide," located in the topic Resources. This document does not need to be filled out, but rather serves as a guide for what to look for in the physical assessment of the community site.

Part II: Interview

Conduct an interview with the appropriate representative selected in Topic 3 regarding that person's role and experiences within the community. Interviews can take place in person, by phone, or via Zoom.

Develop 5-10 interview questions to gather information about the representative's role and the health issues faced within the selected community, the role of public health in addressing environmental issues, and the health issues faced within the selected community.

Part III: PowerPoint Presentation

Based on your interview/community assessment, create a PowerPoint presentation of 15-20 slides (slide count does not include title and references slide) describing the chosen community interest.

Include the following in your presentation:

Summary of how the interview went, including responses to the questions.

Description of the community and community boundaries including the people and the geographic, geopolitical, financial, educational level; ethnic and phenomenological features of the community, as well as types of social interactions; common goals and interests; and barriers and challenges, including any identified social determinants of health. 

Summary of community assessment, including funding sources and partnerships.

Based on your findings from the interview/community assessment, discuss any issues that are lacking or can be improved upon for health promotion, modifying your teaching plan as needed to address these findings. 

A conclusion summarizing your key findings and a discussion of your impressions of the general health of the community.
